Special Event Proposal and License Agreement Form
Any individual or organization wishing to conduct a fundraising event to benefit the Foundation must first complete a special event proposal and licensing agreement form. Once approved by the regional executive director or chapter president, the Foundation will return a signed copy of the agreement form. You may not begin promoting your Special Event until you receive a licensing agreement form that is signed by both the special event group and the Foundation.

Unauthorized Fundraising Methods & Promotions
• Door-to-door or telephone solicitation.
• Use of the Make-A-Wish name at the direct point-of-purchase sale of alcoholic or tobacco products.
• Use of the Make-A-Wish name in relation to any business activity that cannot guarantee full accounting for funds generated, such as vending machines.
• Customer prospect campaigns conducted by independent contractors.
• Promotions to attract new patients to any form of medical treatment.
• Promotions related to the delivery of professional services.

Consult with the Foundation concerning your plans to solicit cash or in-kind donations for your Special Event. This protocol is followed in order to avoid repeated solicitation from generous supporters of the Foundation. Solicitations of any kind are NOT permitted outside the counties the Make-A-Wish Foundation of Greater Ohio, Kentucky & Indiana or via broadcast over the internet. Solicitations are limited to our chapter area and are not to interfere with the efforts and events of other Make-A-Wish chapters.

Budget
The Foundation may also require you to complete the Projected Budget form, depending on the size and scope of your event. The Foundation WILL NOT be responsible for debts incurred by those using the Make-A-Wish name unless prior approval has been obtained by the Foundation.

Insurance
The Foundation requires that all Special Events and Special Event Sponsors obtain and keep in force all necessary insurance, licenses and permits through the conclusion of the Special Event, and comply with all applicable laws during the planning, promotion and conduct of the Event. Check the applicable insurance policy for the specific location of the Special Event or facility where the event will be held to make sure the Special Event has adequate insurance coverage. The Foundation does NOT provide insurance coverage for Special Events.

Liability
Special Events are events which are not conducted by the Foundation and the Foundation cannot assume any liability arising from the event. The Sponsor of the Special Event agrees to indemnify and hold Make-A-Wish harmless from any and all claims of any kind or nature whatsoever arising out of, or in any way related to, the Special Event.

The Special Event Sponsor does not have the authority to act as an agent of the Foundation. The Special Event Sponsor CANNOT open a bank account in the Foundation's name, nor may it endorse, cash or deposit any checks made payable to Make-A-Wish. Any checks received in connection with your Special Event made out to the Foundation should be forwarded to the Foundation as part of the proceeds of your Special Event.

Confidentiality or Contributor Lists
Make-A-Wish contributor lists are not given, leased, sold or exchanged to any individual, organization or company for the purpose of solicitation.

Use of the Make-A-Wish Foundation Name and Logo
The Make-A-Wish name and marks CANNOT be altered in any way. Make-A-Wish is spelled with a capital "A" and with hyphens. Use or reproduction of the Make-A-Wish name or logo in print, electronic or broadcast media is UNAUTHORIZED without PRIOR permission. Mission
Our mission is to fulfill the wishes of children with life-threatening medical conditions. Please DO NOT refer to the children that we serve as "terminally ill" or "dying," as these labels can instill a sense of defeat and can be counterproductive as our wish children fight to overcome their illnesses.

Publicity and Promotional Materials
All promotional materials (including press releases, invitations, tickets, programs, flyers, t-shirts, etc.) related to authorized events and promotions must first be approved by the Foundation's Marketing, Communication & Development department. The sponsoring organization agrees to send samples of all materials using the Make-A-Wish name to the Foundation for approval PRIOR to distribution.

Volunteers
Fundraising Special Events provide an opportunity to involve co-workers, club members, family or friends in the fun and friendship of helping the Foundation; therefore, when you begin to plan your event, first assess your organization's ability to staff the event. The Foundation should be consulted on the feasibility of recruiting Make-A-Wish volunteers to help out.

Wish Family and Celebrity Participation
Make-A-Wish DOES NOT ask wish children and their families to participate in special events. Requests for celebrity participation are reserved for the granting of children's wishes.

Net Proceeds
The Foundation DOES NOT enter into agreements where funds are raised on a net proceeds basis without a good faith (estimated) minimum donation.

All net proceeds of your Special Event will be donated to the Make-A-Wish Foundation of Greater Ohio, Kentucky & Indiana chapter. Within one month (30 days) of your special event, you must provide a simple written accounting, listing expenses and proceeds of your Special Event. The Foundation requires that the Special Event will not result in any cost or expense to the Foundation.

Cause Marketing Promotions
The Foundation DOES NOT endorse products or services; however, it may choose to accept proceeds from cause-marketing promotions by reputable companies which sell products wholesale and/or retail. In each case, a proposal form must be submitted to the Foundation for approval. The sponsoring company must guarantee full accounting of dollars generated by the promotion.


The Make-A-Wish Foundation is a registered charitable organization in the state of its incorporation.
